Certification & Menigitis Acknolwedgement

In order to submit your application, you are required to certify several statements.

• Your certification of these statements serves the same purpose as a legal signature and is binding.
• Once saved select “Continue”.
  
  

Submit

Only completed applications can be submitted.

• The Section Status page will indicate which sections are completed.
• If all sections are completed a “Submit My Application” button will appear at the bottom
  of the page.
• If all sections are not completed, you must return to that section and complete it before a submit button will appear.
